PATIENT PORTAL INSTRUCTIONS

After you schedule your imaging test, be sure to fill out your forms on the Patient Portal any time before you arrive at your appointment.

 

If you have an existing Patient Portal account, please sign in and follow the instructions below.

 

If you have not registered and created a Patient Portal account, please click on "Register for Access" to begin. 

 

You will need to complete 2 sections:

Section One:

On the left-hand part of your screen, click on the Profile tab and complete all of your personal information. 

 

Section Two:

On the left-hand side of your screen, click on the Appointments tab, locate your next appointment, click on the form next to that appointment, complete the forms in full, then submit. 

 

After you are finished:

Please arrive 10 minutes prior to your appointment to complete the check-in process. *If you have been instructed to arrive earlier to receive the contrast liquid, please follow those specific directions.*

 

All patients:

Please bring your ID, insurance card and form of payment to your appointment. 

Acceptable forms of payment are cash, all major credit/debit cards and Care Credit.

We are unable to accept checks.

Note: Using a Lakeside-assigned SID number to complete your forms while at the facility does not set up a patient account. Please set up your account and complete your forms beforehand to access your information in the future.

CANCELLATION & NO-SHOW POLICY

Lakeside MRI & Diagnostic Health reserves the right to charge a $50 fee to any patient who misses their appointment or fails to give 24-hour cancellation notice.

"No-show" fees will be billed to the patient as they are not covered by insurance. They must be paid prior to scheduling the next appointment. Multiple "no shows" in any 12-month period may result in denied service from our practice.

Thank you for your understanding  and cooperation as we strive to best serve the needs of all of our patients. Each time an appointment is missed without proper notice, another patient is prevented from receiving care.

We understand there may be special, unavoidable circumstances which could cause a patient to cancel with short notice. Please let us know if this is the case so that we may work with you.

POLICY OF PAYMENT

Any refunds owed to our patients will be refunded after your insurance Explanation of Benefits (EOB) has been received and verified by our Billing Manager and the Accountant. We will make every effort to issue refund checks within 180 days from the date the EOB was received and verified. This should allow sufficient time for primary and any secondary insurance payers to process claims for payment.

If your EOB determines the allowed amount is more than the amount collected at the time of service, you may receive a bill for additional patient responsibility. Upon receipt of your insurance companies explanation of payment, a statement will be billed to the email address provided at the time of service. Payment is due within 30 days of the statement issued date. Any unpaid balances remaining after 90 days may be referred to an outside collections agency for recovery. At that time, you will be fully responsible for any collection fees and court costs. Thank you for your patience and understanding.
