PATIENT PORTAL INSTRUCTIONS
After you schedule your imaging test, be sure to fill out your forms on the Patient Portal any time before you arrive at your appointment.
If you have an existing Patient Portal account, please sign in and follow the instructions below.
If you have not registered and created a Patient Portal account, please click on "Register for Access" to begin.
You will need to complete 2 sections:
Section One:
On the left-hand part of your screen, click on the Profile tab and complete all of your personal information.
Section Two:
On the left-hand side of your screen, click on the Appointments tab, locate your next appointment, click on the form next to that appointment, complete the forms in full, then submit.
After you are finished:
Please arrive 10 minutes prior to your appointment to complete the check-in process. *If you have been instructed to arrive earlier to receive the contrast liquid, please follow those specific directions.*
All patients:
Please bring your ID, insurance card and form of payment to your appointment.
Acceptable forms of payment are cash, all major credit cards and care credit. We are unable to accept checks.
Note: Using a Lakeside-assigned SID number to complete your forms while at the facility does not set up a patient account. Please set up your account and complete your forms beforehand to access your information in the future.
